2/10/2024 EnchantaVaughn Approved QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her morning. Kaila verbalized to QMHPA that her morning was going good. QMHPA informed Kaila that this session was going to focus on reducing her depressive symptoms. Kaila did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Kaila’s current thoughts, feelings and emotions. QMHPA discussed with Kaila about interacting with family and friends. QMHPA engaged Kaila in a skills building activity. QMHPA explained to Kaila that the purpose of this activity was to teach Kaila ways to interact with peers in an appropriate manner and not just asking for different things from them (i.e. rides). QMHPA questioned Kaila about how she interacts with peers in the community and her definition of a friend. QMHPA discussed with Kaila about depression and things that causes her depression. QMHPA assisted Kaila with calming techniques to assist with helping her depression. QMHPA discussed with Kaila about anxiety and identifying anxiety as well as the symptoms. QMHPA discussed with Kaila different activities he may enjoy to decrease his depression. QMHPA advised Kaila to become more enthused about life and practice on motivational skills. QMHPA explained to Kaila about her behaviors and shutting down on a consistent basis will not help her illness or being able to become self-sufficient. Kaila appeared to be receptive to QMHPA’s interventions and services that were provided to her during the session. Kaila discussed with QMHPA during the skills building activity Kaila indicated that she does not like interacting with people because people always want something from her they are doing things to make her upset. Kaila verbalized to QMHPA that she tried to stay away from her daughter and son because they cause her to become very depressed and stressed. Kaila verbalized to QMHPA that she just do not like people because they try to take her kindness for weakness and she has a bad attitude. When discussing with Kaila about her depressive symptoms, Kaila indicated that she knows what makes her depressed and upset but she continues to allow to engage in the behavior that makes her upset. Kaila discussed with QMHPA that she was going to work on all those triggers and coping skills but it will take time.
